It is easy to enroll with Kelston Medical Centre, just download and fill out the enrolment form and we also need proof of eligibility such as a NZ passport or birth cert.
If you are not a NZ resident would like to know if you are eligible for funded healthcare, click here
Once we receive your form and documents, we will get your medical records from your previous practice. We prefer to have your medical records before you see your new GP so please call to check we have received them.
